About This Tool
Who it's for: Nurse practitioners and physicians.
What it's based on: The 2018 ACC/AHA Cholesterol Guideline (risk enhancers), ATP III criteria (metabolic syndrome), and the AHA PREVENT equations as recommended by the 2026 Dyslipidemia Guideline for formal risk estimation.
How to use it: Enter the patient's data to generate a cardiovascular risk assessment and a referral-routing recommendation — NP-level management, physician or PCP, or cardiology — with the appropriate urgency.

Formal risk should be calculated with the AHA PREVENT equations (10- and 30-year total CVD, ages 30–79) at professional.heart.org/en/guidelines-and-statements/prevent-calculator — the standard recommended by the 2026 Dyslipidemia Guideline, replacing the older Pooled Cohort Equations. This tool provides a category estimate based on risk-factor burden, not a formal percent.
